I use it as demonstrated in the details of the product. I use it mostly on my jawline. It helps make the outline my jawline more visible. It hursts a bit afterwards around my jaws - maybe it’s because I use a bit of force on the fat there. But you have to do it regularly; otherwise the original shape will return.
